Submit Documentation: Fill out the “Q” Form and send it to us along with the medical documentation that is relevant to the procedure that you wish to undergo; case history, ECG, Ultrasound, X-Ray reports, blood test results, urinalysis, etc. You may send the necessary documents to Premium Medicine by mail/post, fax, or electronic mail. This is comparable to face to face consultation that is why we have to charge a consultation fee of $100 USD. The amount is inclusive of a detailed treatment report of the surgery that you wish to undergo. Your payment options include Credit Card or Wire Transfer

4. Appointment Setting & Travel Arrangements: Premium Medicine will be confirming the date which you plan to have the surgery and other pertinent details.

    A. Surgical Appointment: Premium Medicine will be setting an appointment with the surgeon and the hospital where the surgical procedure will be performed.
    B. Visa Info: We will send you all the information you need to get your visa to India.
    C. Travel Options: Premium Medicine will be inquiring about your choices on other important details regarding travel options. We will arrange the flight booking, hotel reservation and airport transfer.

5. Payments Processing: You can process payments via Credit Card or Bank Transfer. The payment to Premium Medicine will be inclusive of all the details that were previously discussed and agreed upon in the previous appointment setting and booking process.

6. Confirmation: Upon receipt of your payment, Premium Medicine will be contacting you for confirmation of all important details regarding the surgery, your travel needs, accommodations, and vacation.

7. Airport Arrival: Premium Medicine will have a representative at the airport to assist you and to accompany you in heading towards your accommodation.

8. Surgery and Recuperation: Premium Medicine will assist you with transfers to and from the hospital including after surgery transfer from the hospital to the accommodation or to the vacation area that you may opt to have a holiday at while you recuperate.
